High Plains Mental Health Center currently has a full time position opening available in our Hays Main Location, for a Health Home Care Coordinator.
Qualifications: The qualified candidate must hold, at a minimum, a Bachelor’s degree in psychology, social work, counseling, or a closely related human services field, have a minimum of one-year relevant experience in case/care management, counseling or working with individuals with serious and persistent mental illness. Must be (1) be 21 years of age or older;(2) possess demonstrated interpersonal skills, ability to work with persons with severe and persistent mental illness and/or severe emotional disturbance, and/or people suffering from chronic physical conditions; (3) have access to reliable transportation; and (4) pass KBI, DCF child abuse check, Adult Abuse Registry and Motor Vehicle screens. Must demonstrate competency in the use of technology including computers, various software applications, printers, scanners, telephones, etc. Must also demonstrate a commitment to continuous improvement, to include understanding and application of technology (hardware, software, equipment and processes).
Summary Description: In accordance with the agency’s Mission, Vision, Values, and Goals, this position provides Care Coordination and Clerical Support for the Health Home Program (HH). In addition, applicant will provide Health Home Care Coordination services individually or in a group setting, assist with tracking ensuring that all required client screenings and reporting measures are complete and submitted as appropriate. Provides support tracking and maintaining HH program population statistics and assists with completion of program enrollment, discharge, and transfer processes. Provides clerical and technical support to the team as appropriate and facilitates review of the monthly Care Management and Hospital Admission reports. Participates in required meetings and trainings, documents all HH activities and reports time to HH Manager.
Applicants should possess a valid Kansas driver’s license and a good driving record with no alcohol related incidents within the last five years. Must also be able to successfully pass an intensive background check including KBI, DCF Child/Adult abuse check and a drug and alcohol screen.
