Position Overview
A liaison nurse serves the hospital community by providing excellent patient care and establishing close points of contact between all hospital staff and the patient. By responding to questions and concerns, and circulating information throughout the hospital, the liaison nurse will create a clear line of communication between various parties.
RESPONSIBILITIES
· Support patients with follow up healthcare post discharge.
· Link patients with community resources.
· Support patients with chronic conditions with medication reviews, follow up appointments and coordination with other healthcare providers.
· Provide suggestions for improved healthcare options with providers, family and patients.
· Provide education to patients regarding their medical care (medications, diagnosis, hygiene, diet habits, etc.)
· Collaborate with other healthcare professionals to ensure coordinated and quality care.
· Assist with insurance questions and completing Medicaid applications.
· Install and maintain Lifeline program.
QUALIFICATIONS & SKILLS
· Current licensure as a Registered Nurse in the State of Kansas.
· Excellent communication skills.
· Ability to create a safe and comfortable environment for patients and staff.
· Willingness to promote health and maintenance programs.
· Close attention to detail.
· Ability to control emotions and remain objective during disputes and investigations.
