High Plains Mental Health center is looking to hire an LPN, Nurse Care Manager for Connections for Life.

New employees will receive a $325 sign-on bonus following successful completion of their 90 day orientation period.

Pay Rate: $21.74 per hour 

  • Monday-Friday; 8:00 AM - 5:00 PM
  • No working Weekends

Qualifications include:
  • Current licensure as an LPN actively licensed to practice in Kansas
  • A minimum of one-year experience in either psychiatric/ mental health nursing or in physician office practice is preferred
  • Must demonstrate competency in the use of technology including computers, various software applications, printers, scanners, telephones, etc.
  • Must also demonstrate a commitment to continuous improvement, to include understanding and application of technology (hardware, software, equipment and processes).
  • Must have a valid Kansas driver’s license and a good driving record with no alcohol related incidents within the last five years.
  • Must also be able to successfully pass an intensive background check including KBI, DCF Child/Adult abuse check, and a Drug & Alcohol screen.

Major Responsibilities:
  • Works closely with the Center’s Connections for Life Manager in the provision of nursing services to the Connections for Life population.
  • The Nurse Care Manager champions healthy lifestyle changes, provides individual care for consumers and facilitates health education activities for clients, staff, and/or community.
  • Facilitates health education activities for clients, staff, and/or community.
  • Coordinates care with internal and external healthcare providers as needed.
  • Attempts follow up on all hospital admissions on their case load, and conducts a medication reconciliation with input from the client’s PCP within 72 hours.
  • Maintains knowledge of pharmacology, health disorders and working with people across the lifespan.
  • Tracks and ensures that required assessments and screenings are completed for each client on their caseload. Includes: Metabolic Screenings/Health Screenings.
