Position: Assertive Community Treatment (ACT) Case Manager
Location: Hays, Kansas | HPMHC
Pay: $25 per hour | + Incentive Pay | $500 SIGN-ON BONUS!
Job Type: Full-time | In-person
Schedule: typical hours Monday-Friday 8am-5pm | On-call rotation

What You’ll Do:

· Provide community-based case management and rehabilitative services to adults with serious mental illness and co-occurring conditions.
· Deliver most services in clients’ homes and throughout the community to promote independence and recovery.
· Develop, implement, and maintain individualized service plans in collaboration with consumers, families, and treatment team members.
· Coordinate care with hospitals, medical providers, community partners, and other stakeholders to ensure continuity of care.
· Support clients in improving life domains including housing, employment, education, social relationships, health, and legal matters.
· Practice assertive outreach to reduce hospitalizations, increase housing stability, and improve overall quality of life.
· Provide mobile crisis response and participate in an after-hours on-call rotation.
· Transport clients as needed and respond to community-based emergencies.
· Complete timely documentation and actively participate in supervision and team meetings.

Qualifications:

· Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
· Demonstrated ability to work effectively with individuals experiencing serious mental illness, functional impairments, and complex needs. Strong interpersonal, communication, and crisis intervention skills.
· Must pass KBI criminal history background check, DCF Child/Adult Abuse and Neglect Registry checks, professional background checks, and complete required state-approved training.
· Valid driver’s license with a driving record consistent with insurability (no substance-related incidents within the past five years).
· Ability to travel regularly (Center vehicle provided) and provide client transportation.
· Proficiency with computers and general office technology.
