Acute Care Discharge Planner

NEW COMPETITIVE WAGES!  Gove County Medical Center has an opening for an Acute Care Discharge Planner.  This position will provide appropriate and timely discharge planning from the hospital, arrangement for follow-up health care, assess financial resources available to patients, and make referrals to appropriate community resources as necessary.  To be considered for this position you must have a state of Kansas RN license in good standing and 2 years nursing experience.  Position schedule will be Monday through Friday, 8 hour shifts.
