Mentor and model how to teach independent living skills to individuals served through consistent implementation of teaching plans/goals and behavior programs. Provide feedback to Personal Assistants for ongoing staff development and quality services.  The treatment of individuals served with dignity and respect is a prime responsibility.  Represent agency with professionalism and advocate for individuals served at all times.

 

Essential Functions

Ensure individuals served are treated with dignity and respect to include using a respectful tone, giving full consideration and attention to each person served and providing opportunities and options that promote independence.  Promote community inclusion of individuals served.
Provide orientation to newly hired Personal Assistants.  Mentor and model newly hired and current employees on how to teach independent living skills to individuals served through consistent implementation of teaching plans/goals and behavior programs.  Provide ongoing training and feedback to promote quality services.  Monitor performance of the staff and report concerns to the supervisor.
Be able to assist in problem solving staffing crisis situations by assisting in locating staff or assisting in covering the shift during staff shortage. Be able to make sound decisions as situations arise and make sure the supervisor is aware of the situation.
Provide assistance with Activities of Daily Living (ADLs) to include support and supervision during meal times, toileting, transition times and transportation, etc.
Required to perform and document medication administration according to DSNWK policy.
Implement crisis intervention strategies as documented in the individual’s life plan. Notify the individual’s team of any new behavior(s) not documented in the individual’s life plan.
Monitor health, safety and ANE issues and report concerns to the supervisor or others as appropriate per agency policy.
Ensure emergency drills and inspections are conducted as required and when required.
Serve as an active team member by attending Life Plans and Life Plan team meetings.  Assist the Coordinator in implementing and training staff on teaching programs, behavioral management programs, Service Agreements and Personal Schedules as outlined in each individual’s Life Plan and provide recommendations for updates as appropriate.
Complete and document a Quality Enhancement (QE) observation at least weekly to ensure quality teaching of skills to persons served to include but not be limited to recognizing opportunities to teach, providing rationales, offering positive reinforcement and encouraging decision making skills by each individual.
Assist in managing and documenting the finances for the persons served spending according to DSNWK policy 3.D.03.  Ensure the persons served have adequate personal items and complete an inventory of persons served personal belongings as per policy 3.D.2.04.
Provide or arrange for the transportation of individuals served to and from desired functions and activities following Life Plan recommendations.
Attend initial and ongoing training in a timely manner to meet the needs of individuals served.
Provide input into and work within the home’s operating budget, to ensure the home has adequate supplies on hand.
Ensure the property is clean and well maintained by developing cleaning schedules and reporting concerns to the supervisor.
Conduct weekly Supervised Living persons served house meetings.
HIPAA Compliance

This position is responsible for maintaining confidentiality for all PHI and ePHI that is shared with them. They will have access to all PHI and ePHI for individuals whom they support as a member of their Life Plan Team or for whom they provide back-up services or on-call supports.
Other Responsibilities

May be required to participate in the on-call system.
Be able to communicate in a variety of modes (i.e. email, chats, T-logs, phone, text, etc.)
Complete other duties as assigned by the Residential/Program Coordinator or Program Manager.
Be familiar with DSNWK policies, forms, and guidelines and where to locate them.
This position is subject to the two year training period as defined by KPERS, unless exceptions are granted due to prior employment.
Job Specifications

Education – High School diploma or equivalent preferred.

Experience – One year of Human Service experience preferred. Prior personal or job related exposure to people with developmental disabilities is preferred.

Extent of physical effort required – Routine job requirements. Frequent physical intervention (i.e. assisted or unassisted lifting and transferring; physical restraint). Must be able to lift 50 lbs. unassisted.

Comments:

Physical requirements – must be physically able to evacuate individuals served in the event of an emergency and be able to apply necessary behavior management techniques as directed by the individual’s Life Plan. Position may have other physical requirements for specific work sites, i.e. climbing stairs, extended periods of standing, etc.

Other Requirements

Valid driver’s license.
Desire to work with individuals with developmental disabilities.
Ability to exercise good judgment and discretion in handling confidential information.
May be responsible for ordering medications and assisting with keeping the medication cabinets stocked and organized.
