Rooks County Health Center (RCH) is seeking a compassionate and skilled RN to serve in a dual-role position as a Wound Care and Outpatient Nurse. This position provides specialized wound care services while supporting outpatient clinical needs in a collaborative, patient-centered environment. The ideal candidate will be responsible for the assessment, treatment and management of acute and chronic wounds, pressure injuries, ostomies and other skin integrity concerns for both inpatient and outpatient populations with the ability to work closely with providers and clinical staff to improve healing outcomes and ensure coordinated care.
Key Responsibilities:
Perform comprehensive wound assessments, staging, and documentation
Develop and implement individualized treatment plans
Provide dressing changes and negative pressure wound therapy (NPWT)
Follow up on newly admitted inpatients and ER consults (during scheduled hours)
Educate patients and families on wound care and infection prevention
Maintain accurate documentation in the EHR
Serve as a wound care resource for staff
Assist as an outpatient nurse as needed and be included on a weekend/holiday rotation.
Requirements:
Current RN or LPN license required
Wound care certification required within 6-12 months of hire
1–2 year nursing experience preferred
Strong assessment, documentation and communication skills
Ability to work independently and as part of a team
Frequent standing, walking, bending, and lifting (up to 50 lbs.)
Ability to work varied hours as needed.
Must pass POET, physical and drug screen
