Care Manager

Post Rock Family Medicine is seeking a caring, flexible, autonomous individual to join our care management team. The requirements for this full-time position include an RN or LPN degree, outpatient clinical nursing experience and familiarity with electronic health records. Preferred applicants will possess knowledge of chronic care management in the primary care setting. Job duties include providing chronic care management services in an existing program, assisting with quality improvement activities including work flow analysis and teaming with existing care management staff for all other care management activities.
