 Administers and coordinates supervised services provided in the individual's home and in the community. Ensures quality assurance practices and all applicable licensing standards and guidelines are met. Ensuring the individuals served are treated with dignity and respect is a prime responsibility.

Essential Functions

1. Assist with the development and monitoring of budget for assigned location(s).

2. Ensure services are in compliance with KDADS, CDDO, KDHE, State Fire Marshall and other licensing standards and agency policy.

3. Represent residential services at life plan meetings and provide input into the coordination of services as well as provide guidance regarding those request that have a financial impact.

4. Hire, supervise and evaluate staff and ensure they are properly trained according to the immediate and ongoing needs of their job.

5. Support DSNWK's Service Delivery Improvement Plan by working toward completion of goals and objectives and reporting progress and outcomes to the Program Manager.

6. Represent residential services and DSNWK with professionalism.

7. Ensure financial obligations of persons served to include period reconciliation of books, cash account, providing access to checking/savings accounts and fiscal planning during the life planning process.

8. Represent residential services at admission/movement meetings and make arrangement for services as needed.

9. Ensured assigned properties are maintained in a manner that is aesthetically pleasing and promotes the health and safety of persons served. Report all repairs and maintenance needs to the Program Manager. Conduct/implement mechanical safety inspections. Conduct or assign monthly drill schedule.

10. Promote community inclusion of the persons served.

11. Responsible for the oversight of residential teaching programs, Service Agreements, Personal Schedules, data collection and Risk Management Plans (when applicable).

12. Participate in the On Call system.

13. Ensure assigned properties maintain emergency supplies in the event of an emergency. If assigned properties are deemed a huddle site, ensure supplies are stored and maintained as outlined in policy.

14. Report health, safety, licensing and ANE issues immediately as per policy.

15. Provide residential supports to individuals served as needed for backup purposes.

16. Complete safety inspections as deemed in the life plan for those persons receiving supported living.

17. Ensure medication administration procedures comply with DSNWK policy and procedure.

18. Ensure ordering and distribution of food bank supplies.

19. Ensure meetings with staff are conducted on a routine basis and meetings of persons served, when applicable, are held at least quarterly with minutes of meetings recorded.

20. Ensure individuals served are treated with dignity and respect.

21. Provide orientation to newly hired staff and provide ongoing training and feedback to promote quality services. Orientation should include but not be limited to the shadow training process and scenario training.

22. Participate and ensure routine Quality Enhancement observation and feedback sessions are provided to direct care staff.

23. Develop, implement and monitor shift schedules.

24. Responsible for shared coordination of medical appointments.

25. Familiarize self with the rights of individuals served and ensure these rights are maintained at all times.

Other Responsibilities

1. Responsible for coordinating residential services, for person served, who are receiving case management and/or day service options, from a provider other than DSNWK, when necessary.

2. Complete other duties as assigned by the Director of Programs and Services and Program Manager.

3. Will have access to all PHI and EPHI for individuals whom they support as a member of their Life Plan Team or for whom they provide back-up services; to provide on call supports; involvement in investigations, as required; and planning for day and residential service options for individuals with dementia and those who are medically fragile.

4. This position is not subject to the two year training period as defined by KPERS.

Job Specifications

Education - BA/BS in a human services field or closely related field preferred.

Experience - Experience in special education in a school system or community based program and management/supervisory experience is preferred. If not degreed, three years of human services experience required with at least one year of management/supervisory experience required.

Extent of physical effort required: Routine job requirements. Moderate frequency of physical intervention (i.e. assisted or unassisted lifting and transferring; physical restraint). Must be able to lift 50 lbs. unassisted.

Comments: Physical requirements - must be physically able to evacuate individuals served in the event of an emergency and be able to apply necessary behavior management techniques as directed by the individual's life plan. Position may have other physical requirements for specific work sites, i.e. climbing stairs, extended periods of standing, etc.

Other Requirements

Desire to work with individuals with developmental disabilities.

Able to exercise good judgment and discretion in handling confidential information.

Valid drivers license.
