First Care Clinic is seeking applications for a Nurse Practitioner.
*Applicants must have current Advanced Practice Registered Nurse (APRN) licensure in the State of Kansas and active Drug Enforcement Administration (DEA) certificate.
The Nurse Practitioner is responsible for providing medical care for patients in collaboration with other FCC physicians within the scope of his/her practice.
Primary Duties:
1. Provide medical care for patients of all ages including an assessment, interpretation of data, and development and implementation of a therapeutic plan.
2. Place a special emphasis on preventative care.
3. Ability to determine when patients require care that is outside of scope and collaborate with other FCC physicians and outside providers as necessary.
4. Follow-up with patients regarding lab results or other medical care concerns.
5. Provide care to nursing home patients as assigned.
6. Meet regularly with the Medical Director to coordinate health care plans and review/evaluate the collaboration efforts.
7. Be aware of existing agencies and resources in the community for patient referrals.
8. Assist and participate in the work of community health projects and educational programs.
