Under the supervision of a medical provider, the Care Manager is responsible for managing a caseload taken from a population of individuals that meet requirements for various quality programs. The Care Manager works closely with patients and their families, clinical support staff, providers, and Quality Nurse to achieve goals of improving clinical outcomes for patients with chronic diseases.
Qualifications:
· Certified Nursing Assistant (CNA) or Medical Assistant (MA) required. Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred.
· Prior experience with care management.
· Must be BLS certified within 60 days of hire.
