JOB TITLE: Health Services Coordinator
SUPERVISOR: Part C Coordinator
EDUCATION & EXPERIENCE: Registered Nurse, B.S.N. required, M.S.N. preferred.
Current Kansas Nursing License.
2 years experience in Community Health Nursing preferred.

JOB FUNCTIONS:
- Conduct health screenings, including health history/health status, nutrition and growth, vision, hearing and social support systems, on infants and toddlers identified as being at risk for developmental delays.
- Assess developmental skills and functional abilities for the purpose of determining eligibility for the Part C/tiny-k program.
- Communicates effectively with community partners, parents, team members for the purpose of providing requested information, developing plans for services, making recommendations and coordinating Part C services.
- Provide Service Coordination for children receiving Part C/tiny-k services as outlined in the Kansas Infant-Toddler Service Procedure Manual.
- Participate in team meetings and appropriate training opportunities as determined and approved by the Part C Coordinator.
- Prepares and maintains a variety of physical and electronic documents, files, and records within required timelines for the purpose of ensuring program compliance with Kansas Department of Health and Environment and federal guidelines.
- Provide direct services to children/families receiving Part C/tiny-k services, assisting parents/caregivers in developing children’s functional skills in all areas of development.
- Monitors children’s progress and adjusts interventions and approaches indicating evidence of planning and progress toward child and family Individual Family Service Plan outcomes.
- Participate in the local interagency coordinating council, representing Part C/tiny-k on standing committee(s).
- Researches resources and methods, participating in continuous improvement/professional development activities and supports new approaches adopted by the Part C team.
- Participate on the Hays Area Children’s Center Policy Committee to develop/update agency policies and procedures.
- Serve as a resource person to Hays Area Children Center and Part C staff for identification/prevention of illness and exclusion from child care or work, when needed.
- Supervise nursing students requiring observation placements as part of nursing curriculum.
- Observe and evaluate environmental conditions that may be harmful to the health and safety of children and staff.
- Provide consultation to child care regarding health and safety regulations from the Kansas Department of Health and Environment.
- Other duties assigned by the Part C Coordinator or as needed by the Child Care Coordinator.
