High Plains Mental Health Center currently has a full time position opening available in our Hays Main Location, for a Health Home Program Manager.
New employees will receive a $325 sign-on bonus following successful completion of their 90 day orientation period.

Qualifications: Current licensure as a RN, APRN, or BSN a registered nurse is preferred. Must demonstrate competency in the use of technology including computers, various software applications, printers, scanners, telephones, etc. Must also demonstrate a commitment to continuous improvement, to include understanding and application of technology (hardware, software, equipment and processes).
Summary Description: To operate and oversee High Plains Mental Health Center’s Health Home Program. Ensures coordination of health home activities to support consumers in the development and maintenance of healthy lifestyles. Ensures supervision and management of Health Home staff. Assists in maintaining agency clinical/regulatory compliance for accrediting/licensing/funding. Evaluates and provides education opportunities to clinical staff and providers as indicated.
Applicants should possess a valid Kansas driver’s license and a good driving record with no alcohol related incidents within the last five years. Must also be able to successfully pass an intensive background check including KBI, DCF Child/Adult abuse check and a drug and alcohol screen.
