Trego County Lemke Memorial Hospital - Long-Term Care is looking for a full-time SOCIAL SERVICES DESIGNEE/ACTIVITIES.
RESPONSIBILITIES
· 1 Full-time position available.
· Participates in coordination with physicians, nursing personnel and other members of the interdisciplinary team in the planning of resident care services and appropriate care plans.
· Participates in arrangements for movement of residents if needed.
· Participates in resident care conferences involving nursing staff and other health team members, as required.
· Demonstrates and promotes an awareness of resident/patient rights; functions as a resident/patient advocate; ensures that staff members are aware of all resident rights.
· Attend care plans.
· Assist with Long Term Care admission process.
· MDS assessment and documentation.
· Help plan social activities, religious programs, creative activities, educational activities, and exercise activities.
· Assist with charting and at times assist with transportation of residents to medical appointments.
· Provides small and large group activities designed to meet the needs and interests of each resident.
· Treats residents and their families with respect and dignity.
QUALIFICATIONS
· High School Diploma or GED equivalent
· CNA certification required.
· Basic computer knowledge, Point Click Care knowledge.
· Prior completion of a State of Kansas SSD certification required.
· Completed a State of Kansas training course designated specifically for this position.
