High Plains Mental Health Center currently has a full time position opening available in our Hays Main Location, for a Nurse Care Manager.
Qualifications: Current licensure as a LPN actively licensed to practice in Kansas to support the Health Home in meeting the provider standards. A minimum of one year experience in either psychiatric/ mental health nursing or in physician office practice is preferred. Must demonstrate competency in the use of technology including computers, various software applications, printers, scanners, telephones, etc. Must also demonstrate a commitment to continuous improvement, to include understanding and application of technology (hardware, software, equipment and processes).
Summary Description: Works closely with the Center’s Health Home Manager in the provision of nursing services to the Health Home population. Coordination of telemedicine services are included in this service. The position requires working cooperatively and effectively with other disciplines. The Nurse Care Manager champions healthy lifestyle changes, provides individual care for consumers and facilitates health education activities for clients, staff, and/or community. Coordinates client care with internal and external healthcare providers and is responsible for tracking and ensuring that all required assessments and screenings are completed for each client as permitted. Utilizes and promotes the use of Health Information Technology (HIT) tools and participates in required meetings and trainings.
Applicants should possess a valid Kansas driver’s license and a good driving record with no alcohol related incidents within the last five years. Must also be able to successfully pass an intensive background check including KBI, DCF Child/Adult abuse check and a drug and alcohol screen.
